You pay national insurance if you earn a certain amount, most goes on your state pension the rest on NHS services that you use, including subsidized treatments, however I would like to bring to your attention that some people pay alot of money for private health care and there is a big difference in where that money goes, so if paying private you would expect something more personal for your money, makes sense really. You get what you pay for!
